About the Aged Care Assessment Officer (Home Support) role

Reporting to the Aged Care Assessment Coordinator, this fixed-term, part-time role (0.5 FTE) involves managing non-clinical Home Support assessments, building personalised care plans, creating tailored support strategies, and maintaining accurate client data on the My Aged Care Portal. The candidate will ensure compliance, meet funding requirements, empower clients' independence, and collaborate to enhance service delivery.

Your Passion and Commitment will see you:
  • Conduct non-clinical Home Support assessments using the Integrated Assessment Tool via My Aged Care.
  • Develop Support Plans to help clients achieve goals and maintain independence.
  • Ensure compliance with standards, maintain data integrity, and meet regulatory requirements.
  • Maintain accurate client data on My Aged Care and meet KPIs for funding.
To succeed in this role you will have:
  • Qualification in aged care, health, or a related field (preferred).
  • Current, valid Victorian driver's licence.
