Case Manager 167 - Edison NJ

Salary: $50,000 per year

Key Responsibilities:

• Provide case management services to individuals with mental health illness, who are at imminent risk/currently homeless.
• The job includes screening, assessment, community outreach across the county, service planning, linkage, counseling, life skills teaching and utilization of HMIS case management software.

Requirements:

• Bachelor’s degree (B.A) from an accredited college or university, preferably in Social Work or a similar Human Service field is preferred.
• At least two (2) years relevant work experience and demonstrated ability to provide effective services to clients required.
• Have a familiarity with housing first, progressive engagement, and reality testing.
• Experience with conflict resolution and mediation.
• Bilingual a plus.
• Must have valid driver’s license and clean driving record.
