Weekly Salary: $1,526.64 - $1,664.03 (plus superannuation)

About the role
The Choices at Home Team Leader is responsible for leading the Choices at Home team to deliver community support activities which help older people live as independently as possible at home, connected to their community. This will include:
- Planning the delivery of choices at home services to older people in line with contractual and budgetary requirements
- Coordinating the delivery of Commonwealth Home Support (CHSP) Services
- Building community awareness of Choices at Home services and increasing usage by seniors seeking entry level CHSP services
- Networking with local NGOs, community organisations, medical centres and allied health specialists to raise the profile of services offered by Choices at Home
- Undertaking client assessment, making appropriate referrals and identifying service gaps by researching the needs of the target group

So what do you need?
To be successful in this role you will need:
- Diploma of Community Services or equivalent
- Demonstrated experience leading a team
- Experience in planning and delivering services to older people
- Highly developed organisation and administrative skills
- Current Drivers Licence
- First Aid Certificate
- Ability to obtain a satisfactory criminal records check
