We are locally owned and operated and committed to providing personalized service and care. We’re looking for an experienced Social Services Director to join our dedicated staff:

• Full-time, salaried position
• Monday-Friday
• SNF experience required
• Licensed Social Worker

Job Description:

Work with families and residents to provide resources and services throughout their stay at The Courvilles. This position requires a person who has a passion for “HumanKindness and Wellness for Life”. Must enjoy working with seniors.

• Manage and coordinate all the major aspects of the patient's care
• Communicate the relevant aspects of the rehabilitation and long-term care process to all necessary parties
• Assist in coordinating necessary and appropriate services
• Assuring financial reimbursement for services
• Promoting adequate documentation

Qualifications:

• Bachelor's degree In social work
• Min 5 years experience in rehabilitation and/or healthcare (Preferred)
• Excellent oral and written communication skills
• Strong organizational and interpersonal skills
• Negotiation skills
• Ability to work autonomously
