Richmond Behavioral Health (RBH) is seeking a full-time Adult Supportive Residential Services Support Coordinator to join our Adult Developmental Services Team. The Support Coordinator will assess client needs, develop, implement and monitor service delivery and person centered plans, coordinate and monitor services with other agencies, implement supportive counseling techniques, directly assist and monitor consumers in their homes and maintain client electronic health records that meet regulatory, reimbursement and agency requirements. This position provides opportunities to work remotely.
Essential Functions
- Provides training and individualized support to individuals with intellectual disabilities who live in their own apartments so they can achieve and maintain community stability and independence.
- Assesses client service needs, capabilities and appropriateness for services; presents options and services based upon a needs assessment.
- Develops a person centered plan of care and services appropriate to the evaluation with the client/family consensus; reviews and updates service plans.
- Makes referrals and linkages to appropriate agencies for services; coordinates client services with multiple service providers and agencies.
- Evaluates the quality of services provided and changes in client’s condition and counsels clients as necessary; evaluates client’s environment for safety, security, and works to resolve problems.
- Monitors activities of daily living, including hygiene and apartment maintenance.
- Coordinates and implements community integration group outings.
- Serves as a liaison to public agencies and landlords providing information regarding Authority programs and services.
- Attends meetings to discuss client plan of care; provides or arranges transportation; coordinates provider attendance.
- Provides crisis intervention and prevention services.
- Prepares a variety of reports; prepares and maintains client charts and other records.
- Attends meetings, as they relate to client, staff and program needs.
