Brooklyn Assisted Living is seeking a reliable and dedicated Home Care Intake Clerk to join our team! In this role, you will be responsible for enrolling members into our home care program and ensuring smooth coordination of services to support our assisted living clients.

Key Responsibilities:

• Enroll new members into the home care program and ensure all necessary documentation is completed.
• Assist assisted living clients in receiving home care services.
• Coordinate services to meet the specific needs of each client, ensuring they receive high-quality care.
• Work closely with families, care teams, and service providers to ensure proper delivery of services.
• Perform local travel to complete necessary assessments and coordinate services.

Requirements:

• Previous experience in healthcare or home care settings preferred.
• Must be comfortable working with senior populations and within a healthcare environment.
• Strong organizational and communication skills.
• Ability to work independently and as part of a team.
• Must have reliable transportation for local travel and in-person assessments.
• Attention to detail and a compassionate, client-centered approach.
