Mds Coordinator/ Rn

The Terrace of St. Cloud is seeking a MDS Coordinator / RN for 120 bed nursing skilled facility. The MDS Coordinator / RN is responsible for supervising, implementing, coordinating, and managing patient care through interpersonal contact with patients, families, nursing staff, and others on his/her respective unit in accordance with all applicable laws, regulations, and company standards.

The MDS Coordinator is responsible for coordinating the accurate and timely completion of Minimum Data Sets (MDS) and related processes to ensure proper individualized care planning and reimbursement in compliance with federal and state regulations. The role serves as a key liaison between clinical, administrative, and interdisciplinary teams to support quality care outcomes and regulatory compliance.

Key Responsibilities

Administrative Functions

  • Coordinate and manage all aspects of the MDS and care planning process.
  • Ensure compliance with OBRA, PPS, and state/federal requirements.
  • Complete and transmit MDS assessments, CAA summaries, and tracking forms.
  • Collaborate with interdisciplinary team members to gather resident data.
  • Analyze quality indicators and identify trends to improve resident care.
  • Facilitate Medicare and PPS meetings; assist with Utilization Review (UR).
  • Educate staff on MDS coding, documentation, and ADLs.
  • Provide accurate, timely updates to all departments involved in resident care.

Committee & Compliance Functions

  • Serve on Compliance, QA, and QAPI Committees.
  • Assist with audits, reports, and care planning agendas.
  • Implement committee recommendations and regulatory updates.
  • Support ethics, discharge planning, and continuous improvement efforts.

Resident Rights & Advocacy

  • Protect resident confidentiality and dignity.
  • Address and resolve complaints or grievances promptly.
  • Ensure resident rights, including the right to refuse care, are upheld.

Work Environment & Physical Requirements

  • Must work throughout the facility, including resident areas.
  • Frequent movement, interruptions, and exposure to stress, infections, and hazardous materials.
  • Must be able to lift up to 35 lbs and assist in emergencies.

Required Education & Experience

  • RN license in good standing.
  • Minimum 3 years of MDS coordination experience.
  • 3–5 years in Long-Term Care required.
  • RAC-CT certification preferred.
  • Strong computer skills; experience with MDS systems (Matrix preferred).
