Physiotherapist ( Nursing Home/ 5 Days/East)

• Assess clients’ physical capabilities, interpret assessment results, and formulate a physiotherapy-based diagnosis

• Collaborate with clients and their families to set treatment objectives aligned with the diagnosis

• Deliver physiotherapy treatments such as exercise programs, therapeutic techniques and patient education

• Monitor and evaluate client outcomes to assess effectiveness and make necessary adjustments to the treatment plan

• Coordinate with healthcare professionals and interdisciplinary team members to ensure a unified approach to client care and goal-setting

• Keep comprehensive documentation of each client’s therapy sessions, progress, and response to treatment

• Support clients’ transition through effective discharge planning or referrals to appropriate services

• Engage in research activities related to physiotherapy practices and innovations

• Design and implement wellness initiatives and physical health screening tools for early detection and prevention

• Oversee rehabilitation assistants, ensuring they deliver interventions in line with treatment plans
