Job Summary
The Program Coordinator/Associate provides day-to-day coordination and administrative support to ensure effective and compliant delivery of services under Maryland DDA waiver programs. This role supports client onboarding, Person-Centered Planning (PCP), collaboration with Coordinators of Community Services (CCS), interdisciplinary teams, families, and staff scheduling to ensure timely, consistent, and high-quality service delivery.
Duties & Responsibilities
· Coordinate client onboarding, intake documentation, and service start-up prior to service delivery
· Schedule and support Person-Centered Planning (PCP) meetings and ensure outcomes are implemented
· Serve as liaison with CCS regarding service changes, authorizations, and compliance needs
· Coordinate with interdisciplinary team members, including RNs and behavioral support professionals
· Maintain communication with families and legal representatives regarding schedules and services
· Assist with service authorization documentation, tracking, and timelines (DSAT, SIPs, schedules)
· Review service notes weekly for accuracy, completeness, and timeliness
· Develop and maintain client activity schedules aligned with authorized services
· Create and manage staff schedules, review timesheets, address coverage needs, and provide on-call support
· Maintain program records, logs, reports, and documentation for audits and reviews
· Support data collection, reporting, and quality monitoring activities
Educational Qualifications
Bachelor’s degree in Human Services, Social Work, Psychology, Health Administration, or a related field.
Relevant experience may be considered in lieu of a degree.
Other Requirements
· 1–3 years of experience in DDA, human services, or working with individuals with intellectual and/or developmental disabilities
· Knowledge of Maryland DDA waivers, PCP processes, and service coordination
· Strong organizational, communication, and time-management skills
· Experience working with interdisciplinary teams, including nursing and behavioral professionals
· Proficiency in Microsoft Office and electronic documentation systems
· CPR and First Aid certification (or ability to obtain)
Pay: $45,000.00 - $52,000.00 per year
