Job Description Summary:
The RN plans, organizes and directs hospice, and plans and directs services to meet the needs of patients and their families within their homes and communities.
Essential Job Functions/Responsibilities
- Completes an initial, comprehensive and ongoing assessment of patients.
- Initiates the plan of care and makes necessary revisions as needs change.
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
- Assists the patient with the activities of daily living and facilitates the patient’s efforts toward selfsufficiency and optional comfort care.
- Acts as Case Manager and assumes responsibility to coordinate patient care.
- Completes, maintains and submits accurate and relevant clinical notes in HomecareHomebase (Pointcare).
- Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
- Teaches the patient and family/caregiver selfcare techniques as appropriate.
- Attends interdisciplinary group meetings.
- Fulfills the obligation of requested and/or accepted case assignments.
Position Qualifications:
- CA RN License
- Compassionate & Caring
- HomecareHomebase (Pointcare) Experience
- CPR certification
- CA Driver License
Schedule:
- Weekdays
- 8 hour shift
Experience:
- Hospice nursing: 1 year (Required)
- Homecare Homebase EHR (PointCare/HCHB) : 1 year (Preferred)
License/Certification:
- California RN License (Required)
- BLS/CPR (Required)
- California Driver License (Required)
