Sp - Nurse Practitioner

UConn Health is looking to hire a temporary Special Payroll Nurse Practitioner who will support the physicians of the practice in the care management and care delivery of existing patients within the Internal Medicine department as required. THIS IS A TEMPORARY POSITION WITH NO BENEFITS. Anticipated end date. 11/30/2025.

SCHEDULE: Part time, 16-20 hours per week, occurring in 4-5-hour time blocks, Monday - Friday, 7:00am - 5:00pm. Work on off shift, weekends and holidays may be required as operationally necessary.

EXAMPLES OF DUTIES:

  • Medication Refills: All refills to be addressed within 72 hours of request, APRN conducting clinically appropriate chart review, including making recommendations, assessment and plan of care, including recommendations for required evaluation and labs for the particular refill, following standards of care and evidenced based practice
  • Patient Calls: APRN to work with clinical support staff in providing telemedicine based clinical triage for patient care; including but not limited to: detailed history of present illness, review of systems as relevant, answering patient questions, making recommendations for next steps and treatment plans, and other duties as operationally required within scope of practice.
  • Patient Messaging: APRN to communicate directly with patients through the MyChart messaging portal, answering patient questions within the required response time of 72 hours, performing chart reviews and collaborating with the patient's PCP when necessary in development of responses.
  • Results: APRN to review all results (laboratory, radiologic, procedural) within their practice scope and specialty, in order of clinical urgency and high priority statuses
  • Timely interventions of critically urgent medical results, including mobilization of emergent care and real-time communication with the patient's care team
  • Formulation of next steps and initiation of care coordination based on result findings, following evidenced based practice and in collaboration with the patient's assigned PCP, including but not limited to: additional testing, referrals, appointments, evaluations, treatment initiations and/or changes to current treatment plans
  • All results review will be documented in the health record in accordance with departmental guidelines
  • Telemedicine: When operationally required, APRN may conduct telemedicine visits for patients, completing required documentation within the health record, billing and coding appropriately, and using critical reasoning skills to evaluate when a higher level of care is clinically necessary or appropriate based on the clinical situation at hand
  • Patient Education: APRN will collaborate with the departmental clinical and administrative leadership in promoting the care initiatives of the department, including but not limited to: documentation of care gaps, motivational interviewing for topics such as weight loss, smoking cessation, controlled substance tapering, among other initiatives as clinically recommended
  • Performs all other duties as required

MINIMUM QUALIFICATIONS, KNOWLEDGE, SKILLS, ABILITIES:
  • Considerable knowledge of anatomy and physiology
  • Knowledge of different disciplines and disease states.
  • Knowledge of medication and their interactions and reactions.
  • Physical assessment skills and counseling skills.
  • Considerable interpersonal skills.
  • Oral and written communication skill.
  • Skill in performing invasive procedures.
  • Ability to work independently; ability to work under pressure; ability to formulate plans of care; decision making ability.

EXPERIENCE AND TRAINING:
GENERAL EXPERIENCE:
  • Registered Nurse and completion an accredited Nurse Practitioner program.
  • Incumbents in this class must possess a valid license to practice in the State of Connecticut

PREFERRED QUALIFICATIONS:

  • Experience at the advanced practice level in an outpatient Primary Care setting
  • Experience as an RN in an outpatient Primary Care setting
  • Outpatient Epic EMR experience

HOURLY RATE: $90
