Position Overview
The Case Manager, MSW plays a critical role in supporting housing stability, program compliance, and successful transitions to permanent housing for up to 15 residents of the Lowcountry Veterans Home (LCVH) transitional housing program.
LCVH operates under a VA per-diem contract and is a non-clinical, community-based housing program. The Case Manager role focuses on case coordination, compliance, documentation, and referral, not the provision of ongoing clinical treatment or medical care.
The Case Manager works closely with the Executive Director, House Manager, VA staff, and community partners to ensure residents meet program requirements and are connected to appropriate external services.
Core Responsibilities
Resident Case Management and Housing Stability
- Conduct intake assessments focused on eligibility, functional needs, strengths, and housing goals.
- Develop and maintain resident-specific action plans addressing housing placement, income and benefits, employment, and access to external services.
- Provide supportive guidance, education, and motivational encouragement to promote independence and successful transition to permanent housing.
- Ensure residents are capable of independent activities of daily living and safe participation in the program.
- Support residents in understanding program expectations, rules, and VA-related requirements.
VA Contract Compliance and Documentation
- Maintain accurate, timely, and VA-audit-ready resident records, including intake documentation, progress notes, referrals, and discharge summaries.
- Track resident attendance, absences, hospitalizations, and discharges in accordance with VA contract requirements.
- Prepare and submit required documentation and reports to the Executive Director and VA staff within established timeframes.
- Monitor compliance with the VA Patient Bill of Rights and applicable confidentiality standards.
- Identify compliance risks or documentation gaps and escalate concerns promptly to the Executive Director.
Screening, Risk Identification, and Referral
- Administer required screening tools, including depression and suicide risk screenings, for referral and compliance purposes only.
- Identify behavioral health, substance use, or medical needs requiring external clinical services.
- Coordinate referrals to VA and community providers for assessment, treatment, or follow-up care.
- Support implementation of VA-led safety plans when applicable.
Collaboration and Coordination
- Maintain professional communication with RHJ VAMC staff, HCHV teams, VA case managers, and liaisons.
- Participate in VA meetings, case conferences, and coordination activities as required.
- Collaborate with community partners to support access to housing, employment, benefits, transportation, and supportive services.
- Coordinate with LCVH staff and approved volunteers to ensure consistent program operations and resident support.
Incident and Crisis Response
- Respond to incidents involving absences, hospitalizations, behavioral issues, or emergencies in accordance with LCVH policies.
- Notify the Executive Director and VA liaison promptly of incidents requiring reporting.
- Coordinate with VA staff, hospitals, and emergency responders as appropriate.
- Support contract-required discharge processes when necessary, including documentation and coordination with VA partners.
Required Qualifications
- Master’s degree in Social Work or closely related field.
- Active South Carolina social work license at the master’s or independent clinical level.
- At least two years of case management experience, preferably with Veterans, individuals experiencing homelessness, or housing-focused programs.
- Working knowledge of VA systems, HUD-VASH, and community-based housing and support resources.
- Strong written documentation, organization, and communication skills.
- Ability to work independently within defined scope and authority.
- Proficiency with Microsoft Office and basic data tracking systems.
Preferred Qualifications
- Experience working directly with Veterans and VA programs.
- Familiarity with VA per-diem or other federal contract environments.
- Training in suicide prevention or crisis response.
- Experience collaborating with multidisciplinary teams across systems.
Additional Requirements
- Ability to work flexible hours, including occasional evenings or weekends as needed.
- Valid driver’s license and reliable transportation.
- Successful completion of background checks and federal compliance requirements.
- Willingness to participate in VA inspections, audits, and quality improvement activities.
- Commitment to the mission of ending Veteran homelessness.
Explicit Scope Clarification
Lowcountry Veterans Home does not provide onsite medical care, psychotherapy, or long-term clinical treatment. The Case Manager role focuses on coordination, compliance, referral, and housing stability, with all clinical services delivered through VA or community providers.

Pay: $33.00 - $39.00 per hour
