Position Summary:

Functions as a member of an interdisciplinary team, responsible for providing case management and primary nursing care to assigned patients. Responsible for coordinating the interdisciplinary team to ensure adequate assessment, planning, and implementation of the patient and family’s plan of care consistent with standards of practice.

Duties and Responsibilities:

Provides primary nursing services for assigned patients in the community.

Provides case management for hospice patients.

Assesses, plans, implements and evaluates nursing plans of care for assigned patients.

Completes all documentation using electronic medical records system.

Maintains contact with patient's primary physicians.

Minimum Qualifications:

Graduation from an accredited nursing program. B.S.N preferred.

Current State of Maryland Nursing License; hospice certification preferred.

Two years of recent clinical experience, preferably with critical care or previous experience in a hospice or homecare program.

Requires strong critical thinking skills and EMR experience.

Possess excellent verbal and written communication skills, and ability to communicate effectively with patients and families under stress.
