Housing Based Case Manager

Position Summary:

The Master Leasing Case Manager plays a key role in supporting youth aged 16–25 through a Housing First model. This position requires innovation, creativity, and a strong understanding of youth engagement to facilitate stable, long-term housing.

In this role, you will:

  • Develop and maintain relationships with landlords to secure units under a master leasing model (OCH signs the lease and sublets to youth clients).
  • Provide comprehensive case management services to support youth in achieving and maintaining housing stability.
  • Work collaboratively with internal teams and external partners to ensure coordinated, wraparound supports.

Reporting:

This position reports to the Housing Coordinator.

Hours of Work:

This is a full-time position, requiring 40 hours per week. Standard hours of work are Monday to Friday, from 8:00 AM to 4:00 PM. In addition, staff are expected to be available to work evenings and weekends as required, based on the needs of the program. Hours may be adjusted with prior notice to accommodate program demands.

Salary & Benefits:

  • Salary Range: $51,480 - $53,310 annually – paid twice monthly

Duties and Responsibilities:

Housing & Landlord Engagement

  • Actively recruit landlords to participate in the master leasing program
  • Locate and secure housing units for youth clients
  • Conduct monthly inspections of leased units
  • Prepare and manage occupancy agreements and related documentation
  • Issue notices related to maintenance, occupancy, or tenant concerns

Client Case Management

  • Conduct client intakes and housing needs assessments
  • Develop individualized service plans based on the Housing First model
  • Support youth in securing and transitioning into housing (e.g., viewings, applications, furniture acquisition)
  • Provide regular home visits and ongoing support to promote housing stability
  • Teach life skills such as budgeting, cleaning, and housing maintenance
  • Advocate for youth on housing rights and connect them to community resources

- Engage in case conferencing, when needed, and collaborate with external parties to promote client success

Documentation & Evaluation

  • Maintain accurate, up-to-date case notes and statistics
  • Input data into internal systems and HIFIS
  • Complete SPDAT assessments and track service levels
  • Participate in discharge planning and transition supports

Collaboration & Teamwork

  • Attend weekly case management meetings and daily team huddles
  • Coordinate with internal staff and external partners for holistic service delivery
  • Contribute to safety planning and crisis interventions when needed
  • Represent OCH in community meetings and networking events

Other Responsibilities

  • Promote OCH programs and services to community stakeholders
  • Respond to inquiries and provide program information as required
  • Attend staff meetings and supervision
  • Perform other duties as assigned by management

Job Type: Full-time

Pay: $24.75-$25.63 per hour
