Mdh@Home Program Coordinator
Mansfield District Hospital is seeking a program coordinator for the MDH at Home program. This program provides aged care services to the community in their homes. It is the only government funded program to care for the elderly in their homes. The successful applicant will coordinate intake, waitlists and funding arrangements with external providers.

You will reporting to the Nurse Unit Manager of Community Care. It is a 0.8 EFT position.
