About the role:
Working hours : Monday to Friday, 9:00 - 17:30; 37,5 hours per week
Contract Type : Full Time, Fixed - Term - Maternity Cover
Location : Usher's Island
Responsibilities
- Completion of needs assessment and development of social care plan with service user to address the social interventions required through key working sessions
- Engage with and build a professional trusting relationship with service users treating them with dignity and respect
- Reconnecting service users with external services and/or making new referrals e.g. counselling.
- Exploring options for accommodation post discharge and after care arrangements/services
- Addressing and managing challenging behaviour
- Monitoring usage of illegal substances in unit. In co-operation with nursing staff, carry out random breath testing and drug screen urinalysis. Carry out random room searches.
- Provide support to service users around court dates and legal information.
- Conduct/facilitate groups which address addiction awareness, relapse prevention, maintaining sobriety, dealing with cravings and high risk situations.
- Co-ordinate the provision of health promotion groups by nursing staff.
- Co-ordinate and monitor Full Time Volunteers, including day to day management of workload, social outings and informal and formal group work.
- Prepare reports, statistics and other information as required.
- Ensure the Pass System is kept up to date.
- Ensure the Service User Data Record Register is completed and kept up to date.
- In conjunction with the Nurse manager and project liaison worker, manage referrals and arrange admissions
- Liaise with recovery with regard to referrals/assessment and, where relevant, move in dates. Prepare for and attend the weekly referrals meeting to ensure that move on to recovery is happening efficiently.
- Liaise with a range of service providers both Voluntary and Statutory (referrals, move-on options, mental health and so on) in order to establish or improve services available to service users.
- Participate in the induction of Full and Part time Volunteers and be a key participant in the induction and ongoing training of new staff, relief staff, graduates, student placements and community employment participants.
- Perform as a full team member supporting colleagues, participating in effective handovers and communicating promptly in order to ensure effective working relationship
- Responsible for the achievement of goals as allocated from unit scorecard
- Attend external/ internal meetings as required. In collaboration with the nurse manager, monitor rent collection and petty cash usage.
Qualifications
Essential:
- A recognised third level qualification to degree level in relevant discipline. Three years experience working in a social care setting.
Desirable:
- Diploma in Addiction Studies. Knowledge of Homeless, and understanding of mental health, addictions, domestic violence and other presenting issues.
