Case Manager
The Care Manager works under professional supervision to coordinate treatment activities, support client care, and implement treatment plans. This role plays a key part in helping individuals navigate both behavioral and physical healthcare systems, while promoting overall wellness and stability.
As healthcare continues to evolve, Care Managers are expected to expand their skills in healthcare navigation, behavioral health interventions, and understanding common medical conditions impacting individuals with mental illness and chronic health needs.
Starting Pay - $22 at hour
Key Responsibilities
- Coordinate care and treatment activities for assigned clients
- Participate in client staffing and implement treatment plans
- Provide supportive counseling and document services using DIAP format
- Conduct assessments and reassessments under supervision
- Develop and update treatment plans in compliance with guidelines
- Collaborate with internal teams, providers, and community agencies
- Maintain accurate and timely client records
- Support clients in navigating healthcare systems and accessing services
- Ensure compliance with all federal, state, and organizational standards
- Maintain clinical productivity and participate in team meetings
- Demonstrate professionalism, cultural competence, and ethical standards
Education & Experience Requirements
Candidates must meet one of the following:
- High school diploma + 2 years of behavioral health experience
- Bachelor's degree (any field) + 1 year of behavioral health experience
- Bachelor's degree in a human services-related field
- Completion of a Behavioral Health Technician (BHT) training program
- Master's degree in a human services-related field (non-licensed)
Preferred:
- At least 1 year of behavioral health experience
- For High Needs Care Managers: 2 years of care management experience + relevant bachelor's degree (or equivalent)
Skills & Qualifications
- Knowledge of behavioral health and care coordination practices
- Understanding of common health conditions and wellness strategies
- Ability to navigate healthcare systems effectively
- Strong crisis assessment and intervention skills
- Working knowledge of DSM-5 diagnostic criteria
- Strong documentation and communication skills
- Ability to work collaboratively in a team environment
Basic Requirements
- Must be at least 18 years old
- Valid driver's license and clean driving record
- Ability to obtain a Level 1 Fingerprint Clearance Card
- Current CPR/First Aid certification (or ability to obtain within 6 months)
- Basic computer skills and ability to manage documentation systems
- Ability to work flexible hours as needed
Work Environment & Physical Requirements
- Frequent sitting, standing, and movement within a clinic setting
- Occasional lifting (up to 15 pounds)
- Ability to work under pressure and manage competing priorities
- Frequent communication in person and via phone
