Case Manager

Better Living Health and Community Services supports seniors, adults with disabilities, and caregivers to maintain independence and quality of life through a comprehensive range of community-based health and support services.

We are seeking a dynamic and compassionate
Case Manager to join our team on a one-year contract. The Case Manager will provide assessment, care planning, case management, system navigation, supportive counselling, education, advocacy, and referrals to clients and caregivers living in the community. The successful candidate will work collaboratively with interdisciplinary teams, healthcare providers, and community partners to support individuals in achieving their goals and remaining safely in their homes and communities.

This position reports to the Manager of Social Work and Navigation Services

QUALIFICATIONS:

The successful candidate will possess:

  • Bachelor of Social Work (BSW) degree from an accredited School of Social Work
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Minimum two (2) years of case management experience in a community health, social service, or healthcare setting
  • Demonstrated knowledge of community resources, programs, and services available to seniors and caregivers
  • Experience working with older adults and vulnerable populations, including individuals experiencing cognitive impairment, caregiver stress, social isolation, and complex health needs
  • Knowledge of relevant legislation, including PHIPA and other applicable privacy and consent requirements
  • Strong assessment, counselling, advocacy, and care coordination skills
  • Excellent interpersonal, communication, organizational, and time-management abilities
  • Ability to work independently in the community and collaboratively within an interdisciplinary team environment
  • Strong documentation and case management record-keeping skills, including the ability to maintain accurate, comprehensive, and timely case notes, assessments, care plans, and service records within electronic documentation systems
  • Successful completion of a Vulnerable Sector Screening within one month of employment

KEY RESPONSIBILITIES:

The Case Manager will:

  • Conduct in-home psychosocial assessments
  • Develop client-centered care plans and implement appropriate interventions based in assessed needs
  • Provide ongoing case management, care coordination, system navigation, and advocacy services to clients and caregivers as outlined in the care plans
  • Assist clients to navigate the healthcare system and community resources by providing expert knowledge of community resources within the North York area
  • Collaborate with primary care providers, hospitals, community agencies, and other service providers to coordinate care
  • Participate in related outreach and educational events as well as in committees both internally and externally
  • Supervise and mentor social work and related students, supporting their professional development and educational objectives
  • Participate in interdisciplinary case discussions, program planning, and quality improvement initiatives
  • Facilitate and evaluate support groups, educational workshops, and community outreach initiatives
  • Develop collaborative, supportive and therapeutic relationships with clients and caregivers
  • Maintain accurate and timely documentation in accordance with organizational standards and professional requirements
  • Be self-motivated and able to function in a fast paced environment
  • Demonstrate solid interpersonal, communication, problem-solving, time management and organizational skills

Salary- $52,000-$57,000 a year, commensurate with experience and qualifications
