Role Impact

As a Hospice Personal Support Worker, you are the steady presence that allows families to transition from 'caregivers' back to 'loved ones.' Through the compassionate care you provide, you ensure that every resident’s final journey is defined by comfort, respect, and dignity. You provide support and care for both the resident and their family members.

Duties and Responsibilities

· Assists with Activities of Daily Living, including bathing, dressing, grooming and oral hygiene

· Assists with feeding and hydration to meet residents' needs

· Performs light meal preparation as required

· Monitors and reports changes in resident status to Registered Staff

· Follows individualized care plan for each resident. Updates care plan and communicates changes to Registered Staff

· Collaborates with the team in planning and delivery of care.

· Provides Registered Staff with updates related to residents' status and abilities for physician rounds and participates regularly in physician rounds.

· Makes referrals to appropriate team members to meet residents' psychological and spiritual needs

· Ensures resident goals of care are communicated to the team

· Ensures families are provided with information on services and programs available to them while at the hospice

· Offers a calm, supportive presence for residents and their families.

· Assists residents and family members with Legacy projects

· Connects and collaborates with volunteers assisting on the floor

· Performs routine housekeeping duties to maintain a clean, organized space for residents, family members and team members.

· Manages time appropriately to complete required daily tasks and asks for assistance from the team as needed

· Adheres to Health and Safety policies and procedures

· Identifies and reports any contravention of Health and Safety standards

Leading Others/Supervisory Duties

· Is a preceptor for PSW Students

· Is a mentor for and provides orientation to new PSW staff members

· Provides guidance and direction to Direct Care Volunteers

Knowledge and Experience

· Graduate of a recognized Personal Support Worker program (Nursing students will be considered)

· Basic computer literacy

· 1 year of palliative care experience

· Police Vulnerable Sector Check required

· Excellent communication skills

· Experience working collaboratively in a multi-disciplinary team

· Committed to ongoing education and learning opportunities

Role Requirements (must be maintained for this role) and Ongoing Education Expectations

· Food Safety Training Certification (must be completed before the end of probationary period)

· Health and Safety Training

· Work towards completion of LIH PSW learning pathway

· Yearly employment required education as assigned in Surge*

Committee Requirements

· Attends Staff Meetings

· May apply to serve on the Joint Health and Safety Committee

· May apply to serve on the Professional Practice Committee

· May apply to serve on the Fundraising Committee

Working Conditions

· 12 hour shifts including days, nights, weekends and holidays

· Travel between sites required

· Work at the hospice is performed in a patient care setting and may involve exposure to waste, gases, blood, body fluids, and hazardous materials.

· Ability to function in the presence of commonly encountered environmental factors:

o Noxious smells

o Disease agents

o Noise and distractions

o Unpredictable behaviours

· Frequent exposure to water and chemicals used in the disinfection/sterilization process

Physical Requirements

· Standing, squatting, bending, pulling, pushing, twisting and repetitive movements

· Lifting up to 30lbs unassisted.

· Ability to provide transfer assistance and mechanical lifting to residents of varying abilities using appropriate body mechanics.

· Combinations of walking around the facility and sitting at a desk, or in front of a computer, for 12 hours

Job Type: Casual

Pay: $23.85-$26.00 per hour
